Pl;ease Email it to  Dr. Walters (preferred) or bring your completed Patient Information paperwork and signed Agreements (in the links below) to your first appointment, along with a current Driver’s License.   You and Dr. Walters will go over the Patient Information and Agreements and sign them.   (This is a good time to ask questions about treatment and expectations for treatment outcomes. Appointments are 45 minutes in duration. If you need more time, let us know when you schedule. Each 15 minute segment is billed over and above the first 45 minutes.)

However, our services may be covered by most insurance policies upon submission to your insurer of your itemized Patient Statement.  We do not take responsibility for the performance of your insurance company.  We accept payment in full at the time services are rendered by checks and cash, OR by Pay Pal remittances made prior to your appointment.
